Coronal Knee Alignment and Tibial Rotation in Total Knee Arthroplasty: A Prospective Cohort Study of Patients with End-Stage Osteoarthritis.

Several studies have found a relationship between the rotational anatomy of the distal femur and the overall coronal lower limb alignment in knees with osteoarthritis (OA). Less is known about the rotation of the proximal tibia, especially in the context of total knee arthroplasty (TKA), where one of the goals of the surgery is to achieve the appropriate component-to-component rotation. The aim of this study was to investigate the relationship between the coronal alignment of the lower extremity and the relative proximal tibial rotation. A prospective cohort study of patients with an end-stage OA scheduled for TKA was conducted. All patients underwent a computed tomography (CT) scan and a standing X-ray of both lower limbs. A relative femorotibial rotation was measured separately for mechanical and kinematic alignment. A statistically significant correlation was found between the tibial varus and the external tibial rotation (p < 0.001). Out of 14 knees with high tibial varus (>5°), 13 (93%) and 7 (50%) knees had >10° of femorotibial rotation for the mechanical and kinematic alignment landmarks, respectively. In order to keep the component-to-component rotation within the 10° margin, more internal rotation of the tibial component is required in knees with higher tibial varus.

Validation of a Robotic Testbench for Evaluating Biomechanical Effects of Implant Rotation in Total Knee Arthroplasty on a Cadaveric Specimen.

In this study, we developed and validated a robotic testbench to investigate the biomechanical compatibility of three total knee arthroplasty (TKA) configurations under different loading conditions, including varus-valgus and internal-external loading across defined flexion angles. The testbench captured force-torque data, position, and quaternion information of the knee joint. A cadaver study was conducted, encompassing a native knee joint assessment and successive TKA testing, featuring femoral component rotations at -5°, 0°, and +5° relative to the transepicondylar axis of the femur. The native knee showed enhanced stability in varus-valgus loading, with the +5° external rotation TKA displaying the smallest deviation, indicating biomechanical compatibility. The robotic testbench consistently demonstrated high precision across all loading conditions. The findings demonstrated that the TKA configuration with a +5° external rotation displayed the minimal mean deviation under internal-external loading, indicating superior joint stability. These results contribute meaningful understanding regarding the influence of different TKA configurations on knee joint biomechanics, potentially influencing surgical planning and implant positioning. We are making the collected dataset available for further biomechanical model development and plan to explore the 6 Degrees of Freedom (DOF) robotic platform for additional biomechanical analysis. This study highlights the versatility and usefulness of the robotic testbench as an instrumental tool for expanding our understanding of knee joint biomechanics.

Femoral Component Rotation in Total Knee Arthroplasty Using a Tibia-First, Gap-Balancing, "Functional Alignment" Technique.

The purpose of this study was to describe the femoral component rotation in total knee arthroplasty (TKA) using a tibia-first, gap-balancing, "functional alignment" technique. Ninety-seven patients with osteoarthritis received a TKA using computer navigation. The tibial resection was performed according to the kinematic alignment (KA) principles, while the femoral rotation was set according to the gap-balancing technique. Preoperative MRIs and intraoperative resection depth data were used to calculate the following rotational axes: the transepicondylar axis (TEA), the posterior condylar axis (PCA) and the prosthetic posterior condylar axis (rPCA). The angles between the PCA and the TEA (PCA/TEA), between the rPCA and the PCA (rPCA/PCA) and between the rPCA and the TEA (rPCA/TEA) were measured. Data regarding patellar maltracking and PROMs were collected for 24 months postoperatively. The mean PCA/TEA, rPCA/TEA and rPCA/PCA angles were -5.1° ± 2.1°, -4.8° ± 2.6° and -0.4° ± 1.7°, respectively (the negative values denote the internal rotation of the PCA to the TEA, rPCA to TEA and rPCA to PCA, respectively). There was no need for lateral release and no cases of patellar maltracking. A tibia-first, gap-balancing, "functional alignment" approach allows incorporating a gap-balancing technique with kinematic principles. Sagittal complexities in the proximal tibia (variable medial and lateral slopes) can be accounted for, as the tibial resection is completed prior to setting the femoral rotation. The prosthetic femoral rotation is internally rotated relative to the TEA, almost parallel to the PCA, similar to the femoral rotation of the KA-TKA technique. This technique did not result in patellar maltracking.

Reference Axes for Tibial Component Rotation in Total Knee Arthroplasty: Computed Tomography-Based Study of 1,351 Tibiae.

Many anatomic landmarks have been described for setting tibial component rotation intraoperatively. There is no consensus as to which axis is best for reducing outliers and preventing malrotation. The SOMA (Stryker Orthopaedic Modeling and Analytics) database (Stryker) was used to identify 1,351 computed tomography (CT) scans of the entire tibia. Several reference axes for the tibia (including the Mayo axis, Akagi line, Insall line, anterior condylar axis [ACA], posterior condylar axis [PCA], lateral tibial cortex [LTC], Cobb axis, tibial crest line [TCL], and transmalleolar axis [TMA]) were constructed according to published guidelines. The Berger method served as the reference standard. The Mayo method (involving a line connecting the medial and middle one-thirds of the tibial tubercle and the geometric center of the tibia) and the Insall line (involving a line connecting the posterior cruciate ligament [PCL] insertion and the intersection of the middle and medial one-thirds of the tibial tubercle) both had low variability relative to the Berger method (7.8° ± 1.0° and 5.1° ± 2.2°, respectively) and a low likelihood of internal rotation errors (0.7% and 1.8%, respectively). No clinically significant gender-based differences were found (<0.7° for all). The same was true for ethnicity, with the exception of consistently greater tibial intorsion in Asian versus Caucasian individuals (mean difference in TCL position, +4.5° intorsion for Asian individuals; p < 0.001). This CT-based study of 1,351 tibiae (which we believe to be the largest study of its kind) showed that the Mayo and Insall methods (both of which reference the medial and middle one-thirds of the tibial tubercle) offer an ideal balance of accuracy, low variability, and a reduced likelihood of internal rotation errors. Setting rotation on the basis of distal landmarks (tibial shaft and beyond) may predispose surgeons to substantial malrotation errors, especially given the differences in tibial torsion found between ethnic groups in this study. Could surgical transepicondylar axis be identified accurately in preoperative 3D planning for total knee arthroplasty? A reproducibility study based on 3D-CT

BackgroundSurgical transepicondylar axis (sTEA) is frequently used for positioning of femoral component rotation in total knee arthroplasty (TKA). Previous studies showed that intraoperative identification of sTEA was not reliable. While surgeons or engineers need to identify sTEA with three-dimensional (3D) computer-aid techniques pre- or intraoperatively, the reproducibility of sTEA identification on preoperative 3D images has not been explored yet. This study aimed to investigate the reproducibility of identifying sTEA in preoperative planning based on computed tomography (CT).MethodsFifty-nine consecutive patients (60 knees involved) who received TKA in our center from April 2019 to June 2019 were included in this study. Six experienced TKA surgeons identified sTEA three times on 3D model established on the basis of knee CT data. The projection angle of each sTEA and the posterior condyle axis on the transverse plane were measured and analyzed.ResultsThe overall intra-observer reproducibility was moderate. The median intra-observer variation was 1.27°, with a maximum being up to 14.07°. The median inter-observer variation was 1.24°, and the maximum was 11.47°. The overall intra-class correlation coefficient (ICC) for inter-observer was 0.528 (95% CI 0.417, 0.643).ConclusionThe identification of sTEA on a 3D model established on the basis of knee CT data may not be reliable. Combined with the previous cadaveric and surgical studies, caution should be exercised in determining femoral component rotation by referencing sTEA both preoperatively and intraoperatively.Level of evidenceIII

Soft-tissue landmarks for tibial baseplate rotational alignment in total knee arthroplasty: A cadaveric study.

The tip of the tibial tubercle (TTT) is used to assess tibial baseplate rotation in total knee arthroplasty (TKA); however, it can be difficult to palpate and visualize intraoperatively. Several more easily accessible soft-tissue structures have been proposed as intraoperative assessments, including the patellar tendon's medial border (MBPT) and the junction of the medial third of the patellar tendon (mt-PT). No studies have described the relationship between the TTT and these proposed landmarks. The aims of the study were to (1) determine the relationship of the soft-tissue landmarks to the TTT and (2) identify any sex differences in these measures. Measurements of the position of these soft-tissue landmarks relative to the TTT were made on 56 cadaveric knees (28 female) by two observers at the level of the standard tibial cut (10 mm distal to the lateral tibial plateau). The results obtained were compared by sex and side. On average, 50.7% (SD 6.79, range 33.1%-63.1%) of the patellar tendon footprint was medial to the TTT. There were no significant differences between the sexes or left and right lower limbs. However, there was large variability in the position of all the soft-tissue landmarks relative to the TTT. The results indicate that, on average, the patellar tendon footprint is evenly spread around the TTT. However, there is a large variability in the anatomical relationship between the soft-tissue landmarks and the TTT. Caution is advised if relying on these structures intraoperatively.

A New Reference Axis for Tibial Component Rotation in Total Knee Arthroplasty: A Three-dimensional Computed Tomography Analysis.

The purpose of this study was to introduce a new reference axis for tibial rotation in total knee arthroplasty (TKA) and verify its reliability. A consecutive series of 80 knees that underwent TKA from 2018 to 2020 as well as 80 healthy knees were analyzed using a three-dimensional tibial model. A coordinate system was established based on the standard TKA tibial cut. The line connecting the lateral-tibial eminence and the medial 1/3rd of the tibial tubercle or the medial border of the tibial tubercle was identified as the lateral eminence line (LE line) and the medial lateral eminence line (MLE line), respectively. To evaluate the reliability of the new reference axis, Akagi's line, the medial third of the tibial tubercle (1/3 line) was compared with the LE and MLE lines by measuring the angle between the lines and the Z-axis. In the coronal view, the intersection angle (TPA), which is composed of the line connecting the center of the medial and lateral tibial plateau with the Z-axis, was measured. The mean angle between Akagi's line and the Z-axis in the healthy group and the osteoarthritis (OA) group was 87.57 ± 3.48° and 87.61 ± 3.47°, respectively. The mean angle between the LE line and Z-axis in the healthy and OA groups was 87.15 ± 4.13° and 86.78 ± 3.95°, respectively. A weak correlation was found between the TPA and Akagi's line and the 1/3 line. A moderate correlation was observed between the TPA and LE lines. There were no significant differences between the healthy and OA groups (P > 0.05) in any of the four reference axes. The LE line showed excellent intra- and inter-observer reliability and reproducibility. The novel and easily drawn LE line is a preferable option for tibial component rotational alignment in TKA.

Accuracy and Validity of Sharma's Venn Diagram Method for Assessment of Tibial Component Rotation in Total Knee Arthroplasty.

Malrotation of the tibial component in a total knee replacement leads to anterior knee pain, patella dislocations, extensor mechanism disruptions, knee stiffness and prosthesis loosening. Techniques like free-floating technique, medial 1/3 rd of the tibial tubercle, medial border of the tibial tuberosity, Akagi's line, transcondylar line of tibia, posterior condylar line of tibia, midsulcus of tibial spines, curve on curve technique have been advocated. None of these have been shown to be accurate and reproducible. We developed a novel 'Sharma's Venn Diagram' method to assess the tibial component rotation. Fifty-two consecutive knee replacements were included in a prospective observational study. The average age of the study group was 53.6years (48-76 years) Thirty-one were females and 3 were males. The patients were followed a minimum of one years (max 2 years, average 1.8 years). 'Sharma's Venn diagram Method (C)' was compared to free-floating method (F) and post-op CT scans using Berger protocol (B). Tibial rotation calculated using Sharma's Venn diagram method (C) coincided with the final component placement in 50/52 knees. The free floating method (F) coincided with method (C) in 30/52 knees with an average 4.8° external rotation in 5 knees and an average of 5.2° internal rotation in 17 knees. Bland Altman method was used to compare method (C) with Method (F), The difference was statistically significant p < 0.0001. Sharma's Venn diagram method is reliable, accurate and easily reproducible by any surgeon performing tkr and correlates with postoperative 2D CT-based assessment of tibial component rotation. Computer-assisted surgery and patient-specific instrumentation improve the accuracy of tibial baseplate rotation in total knee arthroplasty compared to conventional instrumentation: a systematic review and meta-analysis.

To determine whether patient-specific instrumentation (PSI), computer-assisted surgery (CAS) or robot-assisted surgery (RAS) enable more accurate rotational alignment of the tibial baseplate in primary total knee arthroplasty (TKA) compared to conventional instrumentation, in terms of deviation from the planned target and the proportion of outliers from the target zone. The authors independently conducted three structured electronic literature searches using the PubMed, Embase®, and Cochrane Central Register of Controlled Trials databases from 2007 to 2020. Studies were included if they compared rotational alignment of the tibial baseplate during TKA using conventional instrumentation versus PSI, CAS, and/or RAS, and reported deviation from preoperatively planned rotational alignment of the tibial baseplate in terms of absolute angles and/or number of outliers. Methodological quality of eligible studies was assessed by two researchers according to the Downs and Black Quality Checklist for Health Care Intervention Studies. Fifteen studies, that reported on 2925 knees, were eligible for this systematic review, of which 6 studies used PSI, and 9 used CAS. No studies were found for RAS. Of the studies that reported on angular deviation from preoperatively planned rotational alignment, most found smaller deviations using PSI (0.5° to 1.4°) compared to conventional instrumentation (1.0° to 1.6°). All studies that reported on proportions of outliers from a target zone (± 3°), found lower rates of outliers using PSI (0 to 22%) compared to conventional instrumentation (5 to 96%). Most studies reported smaller angular deviation from preoperatively planned rotational alignment using CAS (0.1° to 6.9°) compared to conventional instrumentation (1.1° to 7.8°). Of the studies that reported on proportions of outliers from a target zone (± 3°), most found fewer outliers using CAS (10 to 61%) compared to conventional instrumentation (17 to 78%). This systematic review and meta-analysis revealed that both CAS and PSI can improve the accuracy of rotational alignment of the tibial baseplate by decreasing angular deviation from the preoperatively planned target and reducing the proportion of outliers from the target zone. The clinical relevance is that PSI and CAS can improve alignment, though the thresholds necessary to grant better outcomes and survival remain unclear. IV.

Three Degrees External to the Posterior Condylar Axis Has Little Relevance in Femoral Component Rotation: A Computed Tomography-Based Total Knee Arthroplasty Simulation Study

BackgroundFemoral component rotation in total knee arthroplasty (TKA) has a significant impact on balance and patellofemoral kinematics. However, normal anatomic relationships between rotational axes are poorly understood. As such, we sought to characterize anatomic femoral rotational axes in patients undergoing primary TKA. MethodsWe identified 100 patients who underwent a primary TKA with a preoperative computed tomography scan. The angles between the surgical epicondylar axis (SEA) and the anterior-posterior (AP) axis to the posterior condylar axis (PCA) were measured independently by a musculoskeletal fellowship-trained radiologist and a fellowship-trained arthroplasty surgeon. We simulated an ideal TKA in which the femoral component was placed exactly 3° external to the PCA and measured resulting rotation. ResultsThe SEA was on average 1.5° externally rotated to the PCA (range 3.1° internal to 7.0° external). The AP axis was on average 4.5° externally rotated to the PCA (range 2.3° internal to 10.3° external). The AP axis was a mean 2.7° externally rotated to the SEA (range 6.3° internal to 10.3° external). Routinely setting femoral rotation 3° external to the PCA would result in only 51 (51%) TKAs within ±2° of the SEA and 23 (23%) femoral components internally rotated relative to the SEA. ConclusionNormal anatomic rotational axes of arthritic knees are highly variable, with a 10° range in the SEA and 16° range in the AP axis. Routinely setting femoral rotation 3° external to the PCA will yield significant error in aligning the femoral component with either the SEA or AP axis.

Effect of the presence of the articular cartilage on the femoral component rotation in total knee arthroplasty in female and varus osteoarthritis knees

PurposeSurgical techniques for total knee arthroplasty (TKA) require femoral rotational corrections that alter the position of the surface of the posterior femoral joint especially in kinematic alignment. However, preoperative planning of TKA based on computed tomography (CT), without knowing the femoral cartilage thickness, may cause post-surgery failures in femoral rotation. Therefore, this study aimed to evaluate the effects of posterior condyle cartilage thickness on rotational alignment in the femoral component.MethodsThree-dimensional magnetic resonance imaging (MRI) scans were obtained for 139 male and 531 female osteoarthritis patients. The angles defined by the femoral posterior condylar axis (PCA) and the surgical transepicondylar axis (TEA) were evaluated with respect to the presence of cartilage. Additionally, these effects were evaluated with respect to patient gender and varus/valgus condition.ResultsIn all patients, the angle between the TEA and PCA was significantly greater in the presence of cartilage than in the absence of cartilage. This result was also seen in female patients. However, there was no difference in the TEA/PCA angle in male patients based on the presence of cartilage. The TEA/PCA angle was significantly greater in the presence of cartilage than in the absence of cartilage in the female varus group. However, there were no differences in the TEA/PCA angle based on the presence of cartilage in the male varus/valgus and female valgus groups. Cartilage thickness in the posterior femoral condyle was significantly greater on the lateral side than on the medial side in all and male patients. However, there was no difference between the genders regarding cartilage thickness.ConclusionSurgical planning for TKA based on CT does not consider articular cartilage and could lead to external malrotation of the femoral implant. Therefore, the effect of the remaining posterior condylar cartilage should be considered by surgeons to prevent over-rotation of the femoral component, especially in female varus knees.

Improved femoral component rotation in total knee arthroplasty: an anatomical study with optimized gap balancing.

Surgically balanced total knee arthroplasties have shown improved functional and clinical outcomes. Two different alignment methods have been proposed, the measured resection technique which uses femoral landmarks on the one hand and the ligament balanced technique which uses spreaders on the other. As anatomical landmarks also vary widely, with regards to the tibial cut irrespective of the collateral ligaments, we hypothesized that anatomical landmarks are not suitable for ideal femoral component rotational alignment. Ten cadaveric bilateral knees underwent TKA using a navigational device and a double tensiometer. By means of the navigational device, flexion gaps were balanced by femoral component size, rotation and flexion until a symmetric flexion and extension gap was obtained. Acquired femoral component rotation was compared to femoral landmarks (Whiteside Line, posterior condylar line and trans-epicondylar line). Using the Whiteside line, the posterior condylar line and the surgical trans-epicondylar line to identify femoral component rotation did not balance the flexion gap as well as navigation. Depending on the parameter, deviations in femoral rotation of up to 6° were observed compared to the gap balancing technique. Furthermore, large deviations between these landmarks were observed. Based on this study flexion gap balancing can be better optimized using ligament balancing technique. As this technique is highly dependent on the proximal tibial cut, we do recommend the use of navigational devices, which additionally assure a neutral leg alignment.

Kinematic aligned femoral rotation leads to greater patella tilt but similar clinical outcomes when compared to traditional femoral component rotation in total knee arthroplasty. A propensity score matched study.

External rotation of femoral component in total knee arthroplasty (TKA) with reference to the posterior condyles is recommended in mechanical alignment (MA) to ensure optimum patella tracking. In kinematic knee alignment (KA) technique, femoral component is more internally rotated as the femoral resection is based on flexion-extension axis. This study aims to investigate the clinical and radiological outcomes of the patellofemoral joint in patients who underwent TKA using KA versus mechanical alignment. A review of prospectively collected registry data of 378 consecutive cruciate retaining primary TKAs (P.F.C.® Total Knee System, DePuy Synthes, Massachusetts, United States) was performed. Propensity scoring was performed matching patients who received KA TKA (n = 93) to MA TKA (n = 93). Oxford Knee Score (OKS), Knee Society Score (KSS), Short-Form 36 (SF-36), range of motion as well as radiographs assessing patella tilt were compared at 6 months and 2 years after surgery. All patients received the same implant and had their patella resurfaced. OKS, KSS, physical component of SF-36 and satisfaction rates were comparable at both 6 months and 2 years after surgery. There was moderate association between preoperative and postoperative patella tilt in KA TKA (Cramer's V = 0.260, p < 0.05). Postoperatively, KA group had a greater number of patients with lateral patella tilt compared to the mechanical group (12 [12.9%] vs 1 [1.1%], p < 0.001). Patella tilts, however, resolved two years after surgery. The relative internal rotation of the femoral component in KA TKA results in greater incidence of lateral patella tilt postoperatively. Nevertheless, patella tilt resolution was noted at 2 years. Level III Evidence-Retrospective Cohort Study.

Tibiofemoral rotation alignment in the normal knee joints among Chinese adults: a CT analysis

BackgroundConsensus on tibial rotation in total knee arthroplasty (TKA) remains controversial. The present study aimed to investigate the closest anatomical reference to surgical epicondylar axis (SEA) among 10 tibial markers in Chinese adults.MethodsThis study included examination of 122 normal lower extremities. Briefly, 10 axes were drawn on the axial sections: transverse axis of tibia (TAT), axis of medial edge of patellar tendon (MEPT), axis of medial 1/3 of patellar tendon attachment (M1/3), Akagi line, Insall line, axis of medial border of tibial tubercle (MBTT), and axis of anterior border of the tibia 1–4 (ATC1–4). The mean angles between TAT and SEA and that between other axes and the line perpendicular to SEA were measured. Pairwise differences among the 10 tibial axes were examined by applying one-way analysis of variance (ANOVA) and paired t-test.ResultsIn all the knees, the mean angles of M1/3, Akagi line, Insall line, MBTT, ATC1, ATC2, ATC3, and ATC4 axes were compared to the line perpendicular to the projected SEA and found to be 10.2 ± 5.1°, 1.4 ± 5.0°, 11.9 ± 5.4°, 3.6 ± 4.8°, 12.0 ± 6.9°, 7.2 ± 8.6°, 7.1 ± 10.4°, and 6.6 ± 13.5° external rotation, respectively, and the MEPT axis was 1.6 ± 4.5° internal rotation. The mean angle for TAT was 4.1 ± 5.3° external rotation. The M1/3 and Insall line were significantly more externally rotated than Akagi line, MEPT, MBTT, TAT, ATC2, ATC3, and ATC4 axes. No significant differences were noted between the TAT axis and the MBTT axis and among the ATC2, ATC3, and ATC4 axes.ConclusionThe Akagi line, MBTT, and TAT showed good consistency with SEA in the axial femorotibial alignment with knee in extension. The middle segment of the anterior tibial crest also demonstrated good alignment consistency with SEA for the axial femorotibial alignment. Hence, these markers can be used as reliable references for rotational alignment of the tibial component in TKA.

The Relationships between the Rotational Axes of the Distal Femur in the Arthritic Caucasian Knee: A Magnetic Resonance Imaging Analysis.

Four distal femoral axes have been described to aid in the accuracy of transverse plane component alignment in total knee arthroplasty (TKA). These include the surgical epicondylar axis (SEA), the posterior condylar axis (PCA), and the anteroposterior (AP) axis. Therefore, the primary purpose of this study is to identify the axial femoral relationship with the least variation in a Caucasian population awaiting TKA for osteoarthritis. Secondarily, we further plan to determine if these femoral axis relationships differ with respect to sex or preoperative coronal alignment. These anatomic relationships were measured using preoperative magnetic resonance imaging, which was performed within 2 months of the planned operation date for patient specific instrumentation templating. In terms of our primary outcome, the mean SEA/AP angle was 92.8 degrees (standard deviation [SD]: 2.5 degrees), the mean PCA/AP angle was 95.7 degrees (SD: 2.8 degrees), and the mean PCA/SEA angle was 3.4 degrees (SD: 1.8 degrees). Overall, the PCA/SEA relationship had the smallest variance, while the PCA/AP had the most variance for all comparisons. In terms of our secondary outcome, there was no statistical difference between femoral axis relationships based on preoperative coronal plane alignment. In terms of sex differences, the PCA/SEA was significantly higher in female knees compared with males. Females had a PCA/SEA relationship of 4.7 degrees (SD: 1.3 degrees) compared with 2.6 degrees (SD: 1.2 degrees) for males (p < 0.05). By using the PCA/AP axes, the AP axis was externally rotated by 96.7 degrees (SD: 2.3 degrees) in females, compared with 93.2 degrees (SD: 2.1 degrees) in males, from the PCA. In conclusion, our results demonstrate that the PCA/SEA relationship should be used to determine axial rotation in TKA as it shows the least variation. When using the PCA, approximately 5 degrees of external rotation for Caucasian women and 3 degrees of external rotation for the Caucasian men should be dialed into the femoral cutting block to restore anatomic axial rotation. Further evaluation is needed to determine to role of preoperative coronal alignment on distal femoral axial alignment.

Intra- and postoperative assessment of femoral component rotation in total knee arthroplasty: an EKA knee expert group clinical review.

Malrotation of the femoral component after primary total knee arthroplasty (TKA) is one of the most important problems leading to painful TKA requiring revision surgery. A comprehensive systematic review of the literature was performed to present current evidence on how to optimally place the femoral component in TKA. Several landmarks and techniques for intraoperative determination of femoral component placement and examination of their reliability were analyzed. 2806 articles were identified and 21 met the inclusion criteria. As there is no unquestioned gold standard, numerous approaches are possible which come along with specific advantages and disadvantages. In addition, imaging modalities and measurements regarding postoperative femoral component rotation were also investigated. Femoral component rotation measurements on three-dimensional (3D) reconstructed computerised tomography (CT) images displayed intraclass correlation coefficients (ICC) above 0.85, significantly better than those performed in radiographics or two-dimensional (2D) CT images. Thus, 3D CT images to accurately evaluate the femoral prosthetic component rotation are recommended, especially in unsatisfied patients after TKA. The EKA Femoral Rotation Focus Group has not identified a single best reference method to determine femoral component rotation, but surgeons mostly prefer the measured resection technique using at least two landmarks for cross-checking the rotation. Accuracy of a Novel 3D‐Printed Patient‐Specific Intramedullary Guide to Control Femoral Component Rotation in Total Knee Arthroplasty

ObjectiveTotal knee arthroplasty (TKA) is one of the most universal and effective means for treating terminal stage osteoarthritis (OA) of knee. Accurate intramedullary guide of femur is the basis for the distal femoral cuts. Determining the surgical transepicondylar axis (sTEA) is the key to reconstruction of the femoral rotational alignment, because the correct rotational alignment can place the femoral component in the right position, balance the flexion gap so that the inner and outer tension is equal, get stability during the flexion process of the knee, and enhance the quality of life of patients. With the development of three‐dimensional printing (3DP) technology in the medical domain, the application of patient‐specific instrumentation (PSI) in arthroplasty has become more common. The aim of this study was to evaluate the accuracy of a novel 3D‐printed patient‐specific intramedullary guide to control femoral component rotation in TKA.MethodsEighty patients (65 females and 15 males) with knee OA were included in this prospective randomized study. The patients were divided into two groups by random number table method, 40 in each group. TKA assisted by PSI (PSI group) and conventional TKA (conventional group) was performed respectively. Clinical outcomes [operation time, postoperative drainage volume, duration of drainage, Hospital for Special Surgery knee score (HSS), American Knee Society knee score (AKS)] and radiological outcomes [hip‐knee‐ankle angle (HKA), posterior condylar angle (PCA), patella transverse axis‐femoral transepicondylar axis angle (PFA), depth of intramedullary guide] were compared between and within the two groups.ResultsPSI group had less postoperative drainage volume but longer operation time than the conventional group (P < 0.05). The AKS and HSS scores after surgery were improved compared with those before surgery in each group (P < 0.05). However, there was no significant difference in the duration of drainage and range of motion (ROM) after surgery between the two groups. For the radiological results, the HKA and PFA were improved after surgery in both groups (P < 0.05).The postoperative PFA and PCA of the PSI group were closer to 0°, which was better than that of the conventional group (P < 0.05). The depth of intramedullary guide in the PSI group was less than the conventional group (P < 0.05). But there was no significant difference in HKA before and after surgery between the two groups as well as the preoperative PFA.ConclusionThe short‐term clinical efficacy of TKA assisted by PSI was similar to the conventional TKA. Although TKA assisted by PSI spent more time during operation, it could assist in intramedullary guide and align femoral rotation more accurately.

Accuracy of Reference Axes for Femoral Component Rotation in Total Knee Arthroplasty: Computed Tomography-Based Study of 2,128 Femora.

Many reference axes are used to evaluate rotation of the femoral component during total knee arthroplasty, including the Whiteside line, surgical transepicondylar axis (sTEA), anatomical transepicondylar axis (aTEA), posterior condylar axis externally rotated 3° (PCA+3°ER), sulcus line, and femoral transverse axis (FTA). There is no consensus about which of these axes is most accurate. The Stryker Orthopaedic Modeling and Analytics (SOMA) database was used to identify 2,128 entire-femur computed tomography (CT) scans. The Whiteside line, aTEA, PCA+3°ER, sulcus line, and FTA were constructed according to published guidelines. Every axis was compared with the sTEA, which is widely regarded as the gold standard reference axis for rotation of the distal part of the femur but has low intraobserver and interobserver reliability intraoperatively. The PCA+3°ER differed from the sTEA by a mean (and standard deviation) of 0.60° ± 1.64°; it was the most accurate but also had the highest degree of intersubject variability. The mean PCA-sTEA angle was 2.40°, close to the accepted "rule of thumb" of 3°. This value was significantly higher in women (2.64° ± 1.74°) than in men (2.18° ± 1.52°; p < 0.001). The Whiteside line differed from the sTEA by a mean of 1.90° ± 1.38°, and the sulcus line differed from the sTEA by a mean of 1.94° ± 1.49°; neither of these values varied significantly with sex or ethnicity. The FTA differed from the sTEA by a mean of 2.04° ± 1.50°. Least accurate was the aTEA, which differed from the sTEA by a mean of 2.05° ± 1.33°. The combination of 3 axes that are readily available intraoperatively (the Whiteside line, aTEA, and PCA+3°ER) differed from the sTEA by a mean of 1.80° ± 0.70°. In the largest study of its kind, analysis of CT scans of 2,128 femora revealed that no 1 axis could serve as a marker of femoral component rotation with both high accuracy and low variability. Utilizing a combination of 3 methods (PCA+3°ER, the Whiteside or sulcus line, and aTEA) to maximize accuracy and sex and ethnic generalizability when positioning the femoral component is recommended. A large-scale study using a CT-based biomorphometric database demonstrated that use of a combination of 3 axes (PCA+3°ER, the Whiteside or sulcus line, and aTEA) was the optimal strategy for judging femoral component rotation.

The Effect of Femoral Cutting Jig Design on Restoration of Femoral Offset in Posterior-Referenced Total Knee Arthroplasty.

Femoral component sizing and rotation in total knee arthroplasty (TKA) affects patellofemoral tracking and tibiofemoral mechanics. Posterior referencing is said to optimize restoration of posterior condylar offset. However, it typically allows the surgeon to select 3° to 5° of external rotation to the posterior condylar axis, inevitably changing the offset of one or both condyles. The axis about which external rotation occurs varies between the jigs of various TKA systems. The location of this axis can result in a medial, central, or lateral reference point for posterior offset restoration. Variations in jig design will result in varying posterior offset changes at the same jig setting, with differential effects on balance between the flexion and extension gaps. Using identical Sawbones in a controlled laboratory setting, 9 TKA instrumentation systems were examined. Two systems referenced medially, 1 referenced laterally, and 6 referenced centrally. The authors measured distal and posterior resections in both 3° and 5° of external rotation to the posterior condylar axis. They calculated changes in both distal and posterior joint lines using resection measurements and implant specifications. Posterior resection thicknesses were greatest with instruments that referenced laterally and least with instruments that referenced medially. With increasing external rotation, instruments that referenced off the lateral femoral condyle introduced the greatest mismatch between the distal and posterior joint lines, as compared with instruments that referenced centrally or medially. Surgeons should be aware that laterally referencing systems can differentially restore distal and posterior joint lines at higher settings of femoral external rotation, potentially introducing incongruity between flexion and extension gaps if the posterior slope of the tibia is maintained. This may be particularly problematic if flexion laxity is increased by posterior cruciate ligament sacrifice. [Orthopedics. 2019; 42(6):316-322.].
